PLAYER REGISTRATION

 

 

If you are interested in playing for the Colorado Basketball Club you must complete the following forms prior to tryouts or practice attendance.

 

1. Membership Application 

2. Medical Release Form

3. Credit Card Form

 

All players must have these forms on file with Colorado Basketball Club.  All files will be sent electronically and are protected.  For your convenience you may send the forms to the address below:
