If you are interested in playing for the Colorado Basketball Club you must complete the following forms prior to tryouts or practice attendance.

 

1. Membership Application – (Player information Form)

Include a copy of the players birth certificate

 

2. Liability Waiver Form

 

3. Medical Release Form

 

4. Media Release Form

 

5. Player Profile Web Release form

 

6. Code of Conduct
